1:1 College Meeting Sign Ups

Please use the links below to schedule a 1:1 meeting with your assigned counselor if you are applying to any university that requires a counselor letter of recommendation. If you have trouble finding an available date, please email your counselor to schedule an appointment.

The following tasks are REQUIRED to be completed prior to your 1:1 meeting:

If your colleges are not requiring a counselor letter, schedule a meeting with Mrs. Preston. Note: UC/CSU/community colleges do not require a transcript or letters of recommendation.

Transcript requests

Directions: Follow the instructions below based on how and where you need your transcript sent. Complete this form once you have reviewed the transcript delivery options below. Transcripts will be sent by the application deadline. We require 5 school days’ notice to send transcripts. You may check Naviance for submission status updates.

  1. Electronic Delivery to Colleges Listed in Naviance (Colleges I’m Applying To)
    • Add college(s) in Naviance under “Colleges I’m Applying To”
    • Check Naviance: there must be a blue computer symbol showing under “Submission Type” in the “Colleges I’m Applying To” section or else we can’t send electronically through Naviance and you must request another option
    • Make sure deadline and delivery type is correct in Naviance
  2. Mail Delivery to a college
    • Must provide a pre-addressed and stamped envelope to Ms. Alba, OPHS Registrar, in the main office.
    • Make sure to attach your first and last name somewhere on the envelope (e.g. post-it, paperclip) that you provide Ms. Alba.
  3. Email or Upload to a college NOT in Naviance
    • See Ms. Alba at the front desk or contact her via email at kalba@opusd.org.

Mid-Year Reports

If one or more of your colleges requires a mid-year report, you must complete this Google form by January 17, 2025. A mid-year report is a form submitted by your counselor indicating your academic progress for the first half of your senior year. Along with the mid-year report, your counselor will submit a mid-year transcript.
